In anime or in real life? I've found in real life it's pretty distinctive, even if you don't have the context.

It's an anime, actually, so I think you're right: if it were a real person--even an actor--it wouldn't be awkward/ambiguous.
The problem could also be me; I don't have a very large window to take the screenshot in before he
does
pass out.
