Stein is certainly not the only character in manga who has obsessive thoughts about harming others and suffers from hallucinations yet is still a big damn hero, but he's one of the best-written ones out there. He's not only a hero, but he's an important mentor to most of the main cast. (Remember how Maka ran and hugged him when he came back from being a fugitive?)
I thiiiiink Ohkubo said in an interview somewhere that Stein was his own favorite character because he himself often has obsessive thoughts about blood and cutting (himself or other people), but my link-fu is weak right now so maybe somebody else can dig that up...?
